Despite the killer himself being the central reason he was sent back in time, the introduction of the girl as a central character whom the protagonist interacts with extensively solidifies her role in the eyes of the reader as "the love interest", fulfilling the trope of the childhood friend-turned-lover later in life. It sets up a narrative payoff, builds on it over the course of the story, but at the last moment it flips the script and doesn't pay out.
Even if it was never the author's intention to pair them together, the way the story is written as well as the promotional material for the series heavily implies that she is the main love interest for the main protagonist.

It's a bit of a narrative bait-and-switch.

The issue is he was a flip flopping half-measure pussy that didn’t do his job well. The manga could have landed so much better, and making the ship work wasn’t even the only way to do it. It’ll just have to remain a flawed and interesting manga with a weak conclusion I guess. So it goes.

I think the relationship between Satoru and Kayo has more elements than the purely romantic, and trying to reduce it to just a dropped romance route is a disservice to both characters.

You set something up, you’ve created expectations in the audience. You can either validate those expectations, or subvert them. If you choose to subvert them, it should either be something they didn’t even know they wanted, or to make a big point about what they wanted. Instead the mangaka decided to write this aspect out of the story as abruptly as physically possible, in service of a twist ending that wasn’t even good Kayo aside. It wasn’t an absolute 0/10 disaster of an ending in the sense that the hero beat the villain and got a hopeful ending of his own, but it absolutely chose to intentionally avoid a major dramatic question of the meat of the manga, and essentially sever the main relationship (not just romantic) to do it.
