If that was the case they could both pretty easily exist, with Tooru fucking off when the head doctor had anything important to do then putting him on autopilot when Tooru was moving around. I thought that was implied when the receptionist said that the head doctor was old and had bad hearing, like people would try to get his attention but he was on auto-pilot and ignored them.

For me its more about arcs not being narratively consistent. When he chooses them to be silly inconsequential arcs like Milargo man/Shakedown road or serious plot progression arcs like Vitamin C they are great. However most of the arcs are in a narrative limbo where they are serious but once they end they have zero consequences to the next arc (This hurts vitamin C too imho). I think the main reason for this is Araki treating Morioh as a comfort setting where he can do whatever he wants. But this just comes as non-commitment to the narrative.

To give Jojolion credit, I have to respect Araki's decision of including the rock humans/animals as a reocurring threat. The franchise started with vampires and "pillar men", but part 3 onwards completely drops the supernatural creatures in favour of stands. DIO's vampirism is outshined by The World, Baby Face is a stand and the Green Baby is the closest we get to another relevant creature, only for it to fight solely using a stand. Even Part 7 had a hamon equivalent, but none of the "hamon energy/vampire energy" dynamic. While the rock humans' biology didn't alter the fights whatosever (I assume Araki originally planned to, with the architect getting tapes from his eyes), the rock creatures are easily among the most unique and bizarre enough to stand out, all parts consideres too
