imo Johnny had the worst childhood because Dio chilled out temporarily and started pretending to be nice after killing Danny. I think Johnny dealing with the guilt from his brother's death and his abusive father, plus getting crippled (partially his own fault) was worse.
On the other hand he suffered the least in his adulthood since his only major trauma was Gyro's death, and although I'd say Gyro and Johnny's friendship is much stronger than Will and Jonathan's, Jonathan had to deal with a bunch of other shit on top of that. His sacrifice to save his wife and son is also comparable to Jonathan's sacrifice to save pregnant Erina and Lisa Lisa.
