Kyosuke would have been a completely different person if he had to confront losing his lifelong passion. That's where the wish comes from in the first place.
So yeah, without the wish in the equation Sayaka would have made her move, I'm fairly sure. Then the question is what if the injury happens vs. what if it doesn't? If it does then Kyosuke would become much harder in having a relationship vs. if he doesn't have to deal with it.
