In all seriousness, in my 21 years or so of watching anime since I was a tween, I've never seen any actual sci-fi isekais. Where are they? Why is it nearly always medival europe or japan with a generic fantasy coat of paint?

Few reasons, I think.
1. Isekai usually involves MC having knowledge no one else has. That is easy to believe if world's tech is medieval. But impossible if the tech is sci-fi.
2. Most of isekai give MC a personal power-up, that comes from within, eg. magic. In almost all sci-fi, power comes from external tools and technology. More difficult to write a unique MC when he is forced to use same gear as everyone else.
3. Current zeitgeist sees future and sci-fi as cynical and dystopian. Most isekai try to go with idealistic image.
4. Most isekai show MC as person doing the adventuring and fighting. But in most sci-fi, MC is stuck in spaceship or mech or power armor, making the image less personal.

Mars is technically an isekai if you think about it
