I still find it funny how all the tourists were freaking out and caught off guard by the incest kiss. I think the opposite is true actually. From the start Ruby's whole character arc was likely planned to be in conflict about being in love with the doctor, who is now her twin brother. The pieces are laid out pretty clearly in the beginning. But someone told him no incest, so after that he really didn't have a plan for Ruby, so she was mostly put on the back burner for the majority of the plot.

It was selfish, but it's not because he decided to fulfill his revenge.His last thoughts are about how he was reborn to protect Ruby, and that he is happy he gets to die before Sarina this time. That was his ultimate goal, and he succeeded. It was selfish because he threw away his own life in the process of doing so, knowing that it would ultimately leave Ruby alone.
